As individuals leave Transitional Care programs, efforts are made to place them in a supported housing arrangements using our Ready–Set – Go Model, and referrals are made to local mental health centers to ensure continued support and treatment.

Extended Care Programs

The Extended Care Programs provide treatment and housing for 203 individuals and include Group Homes, Adult Foster Care Homes and Forensic MOM Apartment Programs. The Group Homes and MOM apartments are staffed and operated by JBS. Individuals in Group homes participate in Rehabilitation Day Programming activities that emphasize an educational and rehabilitative approach to recovery.

Adult Foster Care services are subcontracted with individual providers certified through the Alabama Department of Mental Health. Individuals in Adult Foster Care  receive case management services and clinical services via the local community Mental Health Center in their catchment area.

JBS also owns and operates four, two-bed Townhomes for independent living. Although no resident manager is located on the premises, case management services are provided to all residents.

Additionally, JBS operates 20 scattered site apartments for individuals who are both homeless and experience a mental illness. Case management support is provided as well as Day Treatment activities.
